of individuals had multiple online medical records or patient portals in 2024.
U.S. Office of the National Coordinator for Health IT ↗Your care shouldn’t
restart at every step.
CareLoop connects coverage, appointments, clinical notes, costs, medications, authorizations, and follow-ups into one understandable patient journey.
Patients have become the
integration layer.
Healthcare is a chain of handoffs. Records, coverage, appointments, prescriptions, and next steps live in different places—leaving the patient to carry context between them.
Fragmentation is not an edge case.
used an app or portal that organized records from multiple sources.
ONC patient portal data ↗of Medicare beneficiaries saw five or more physicians within one year.
Medicare outpatient care trends (2019) ↗HL7’s FHIR standard also recognizes that distributed systems can hold separate, overlapping, and potentially contradictory records for the same patient. Read the specification ↗
Follow a sample patient through CareLoop.
At every step, CareLoop answers three questions: what happened, what is waiting, and who acts next?
Your coverage, in plain language.
Estimate only. Confirmed from the plan snapshot on file.
In-network care for a specialty follow-up.
A clinical draft the patient can understand.
Increased thirst and fatigue since the last visit.
Current medication and prior history captured.
HbA1c test and medication review suggested.
Draft only—nothing becomes final without clinician review.
Everyone knows what they own.

From scattered steps to one story.
Without CareLoop
- Information across multiple portals
- Unclear in-network status
- Medical language without context
- Uncertain costs and next actions
- History repeated at every visit
With CareLoop
- One longitudinal patient thread
- Coverage context before booking
- A patient-readable visit summary
- Clear estimates and ownership
- A shareable history for future care
Every integration has
a specific job.
CareLoop combines multimodal AI with eligibility, authentication, and deterministic healthcare logic. Here is what powers each part of the demo.
Insurance documents
grok-2-vision-1212 reads uploaded insurance cards and benefit summaries into structured JSON for the patient to review.
Visit transcription
grok-voice-transcribe-2.0 turns recorded visit audio into a transcript that feeds the draft visit summary.
Structured care drafts
grok-2-1212 produces structured visit drafts and prior-authorization and appeal letters. For a visit cost estimate it may use only the CMS DE-SynPUF 2008–2010 carrier line allowed-charge figures supplied in the request — a synthetic public use file, not a bill and not current Medicare payment. Patient-owed copay, deductible, and coinsurance stay deterministic. A missing key, a failed call, or a code with no DE-SynPUF rows uses the demo fee schedule instead.
Eligibility checks
A test-only 270/271 flow demonstrates real-time coverage verification and maps the response into CareLoop’s patient-friendly coverage view.
Identity & profiles
Supabase Auth handles live signup and login, while public.profiles stores the matching patient profile. Service credentials remain server-side.
Resilient text generation
openai/gpt-oss-20b is an optional text-only fallback for generation requests, including letter drafts, when the primary xAI chat request is unavailable.
Extractive summaries
LexRank creates transcript-grounded summary bullets without an LLM. Specialty suggestions and denial risk use deterministic Python rules. Patient-owed visit amounts do too; the allowed-charge range is the DE-SynPUF path above, or the demo fee schedule when that path does not run.
Product runtime
One Python 3.12 FastAPI service exposes the APIs and serves the framework-free patient interface, deployed as a Vercel serverless app.
Trust is a feature.
✓ No autonomous diagnosis
✓ No invented eligibility or copays
✓ Estimates are clearly labeled
✓ Clinician review before finalization
✓ Human approval before letter download
✓ Synthetic patient data in the demo
Functional depth, honestly labeled.
Patient journey
Coverage intake, clinician search, booking, visit flow, SOAP draft, costs, care plan, and history.
Human-reviewed letters
Prior-authorization and appeal drafts with risk scoring, watermarks, and approval gates.
External connections
Sandbox eligibility and AI extraction demonstrate how real systems can connect safely.
From understanding the journey
to coordinating it.
Pharmacy checkout
Partner with CVS and other pharmacies to securely share clinician-approved prescriptions and enable easier ordering, delivery, or pickup.
Insurance claims agent
With patient authorization, communicate with insurers, explain status, gather documents, track actions, and prepare responses for review.
Appointment agent
Find in-network providers, compare availability, schedule or reschedule visits, prepare documents, and coordinate follow-ups.
Automated check-in
Automate check-in across hospital and physician websites or digital intake forms, eliminating repetitive paperwork and clipboard friction.
Insurance tie-ups & discounts
Partner directly with insurers to unlock pre-negotiated visit rates, member-exclusive discounts, and lower immediate out-of-pocket costs.
Multi-factor security
Incorporate biometric and hardware-backed multi-factor authentication (MFA) to strictly safeguard patient health records and coverage profiles.
